A qualification that provides an understanding of how care plans are developed, implemented, and reviewed to meet individuals’ needs. It covers person-centred planning, assessing needs, setting goals, and monitoring outcomes to ensure effective care and support. The course is suitable for individuals working, or preparing to work, in health and social care roles who are involved in supporting or contributing to care planning.

The qualification will be assessed by a portfolio of evidence. Your personal assessor will conduct monthly meetings via Teams to review and develop your progress towards the qualification. | All assessments need to be passed, and each will directly assess the knowledge, skills and behaviours of the qualification.
